How to use this pad

  • Use this tear off notepad to discuss why your diabetic foot patient has a wound, by circling the areas that is specific to them. The most common issues in diabetic foot disease is diabetes, immune system, loss of feeling, deformity, footwear, blood supply and increasingly kidney disease.
  • You can write in any language or just leave as pictures for non english speakers.
  • Those that cannot read can understand the meanings of the icons.

Use this for patient notes. Jot down a summary of their issue and last treatment plan, and what follow up was to be done by this consult.

Did you check their bloods, swab, xray, scan and what were the results? Do you have to chase it up before they come.

Jotting down and then discussing with the patient helps me remember what to do and makes the patient think wow they really know my case, even though you may not have seen them before.
